Rapidly expanding aging service provider seeks a full time Case Managers/Care Coordinators for its Aging Services program in Coles and Edgar counties

Duties and Responsibilities:

• Performing in-home comprehensive assessments,
• Performing hospital and LTC facility based assessments, including week-end rotation
• Developing, monitoring and revising person centered plans of care
• Providing linkage and referral to other social service organizations
• Follow up activities, etc.
• Adherence to program timeframes and policies a must.

Qualifications and Skills:

• Candidate must possess a bachelor's degree in social science or health field or be a registered nurse
• Master's degree preferred
• Position works out of the Charleston office
• Extensive travel and dependable transportation with vehicle insurance required
• Valid Driver’s license
• Computer literacy
• Strong organizational skills and the ability to work independently
• Must be fully vaccinated, including boosters or provide ADA acceptable documentation verifying reason for non vaccinated status
• Bilingual candidates preferred
