Overview:
We are seeking a highly skilled and compassionate Chronic Care Manager to join our team. As a Care Manager, you will play a crucial role in providing comprehensive care coordination and management for patients. This position requires expertise in patient assessment, diagnostic evaluation, critical care, and managed care.
Responsibilities:
- Conduct patient assessments to determine their healthcare needs and develop individualized care plans
- Coordinate and monitor the delivery of healthcare services to ensure quality and continuity of care
- Collaborate with healthcare professionals to develop and implement treatment plans
- Provide education and support to patients and their families regarding their healthcare needs
- Conduct diagnostic evaluations and recommend appropriate interventions
- Ensure compliance with HIPAA regulations and maintain patient confidentiality
- Advocate for patients' rights and ensure they receive appropriate care
- Monitor patient progress and adjust care plans as needed
- Collaborate with interdisciplinary teams to ensure effective communication and coordination of care
- Provide support and guidance to other healthcare professionals
Requirements:
- Valid nursing license ( LPN)
- Minimum of 1 years of experience in a clinical setting, preferably in critical care or managed care
- Excellent assessment skills and ability to make accurate diagnoses
- Knowledge of managed care principles and practices
- Proficient in using electronic medical records systems ( Athena a Plus )
- Strong communication skills, both verbal and written
- Ability to work independently as well as part of a team
