    JOB DESCRIPTION SUMMARY

    The LVN Clinical Support is responsible for supporting the Clinical Supervisor in ensuring that care is coordinated, services are delivered, and care managed appropriately. The LVN Clinical Support assists the Clinical Supervisor in supervising all clinical nursing staff (RNs, LVNs, HHAs) and provides training, mentoring and coaching as delegated.

    Position Qualifications

    • Licensed Vocational Nurse (LVN) with current and unencumbered license to practice professional nursing in the state of employment.
    • Associate degree in Nursing from an accredited program.
    • One (1) or more years of home health care experience required.
    • Possess and maintain valid CPR certification. Must have obtained in-person with a practical skills evaluation as part of the final test.
    • Working knowledge of OASIS documentation requirements, required.
    • Complies with accepted professional standards and practice.
    • Demonstrates excellent observation, verbal and written communication skills.
