To be a Clinical Nurse Specialist typically requires 5 years experience in the related area as an individual contributor. 1-3 years supervisory experience may be required.     JOB TITLE: CLINIC  TRIAGE RN

    JOB PURPOSE: 

    Perform clinical duties under the direction of a KSM provider and/or Nurse Manager by:

    • Possessing an in-depth knowledge of KSM clinic procedures and protocols, along with knowledge of patient care procedures, lab procedures, electronic medical record documentation, medical terminology, and sanitation practices.  
    • Working within the RN scope of practice

    JOB DESCRIPTION:

    1. Provide patient-centered care
      1. Respond promptly to incoming calls.
      2. Apply the nursing process for calls requiring triage, and consult a KSM provider as needed.
      3. Receive and manage provider orders for labs, diagnostic procedures, and referrals.
      4. Evaluate medication requests and send refills electronically to pharmacies as allowed by KSM protocol and under the direction of a KSM provider.
      5. Analyze all incoming lab and diagnostic results, address abnormal results, notify the provider appropriately, obtain new orders and follow up with patients.
      6. Educate patients regarding abnormal test results and/or medical conditions, and provide education materials to patients as requested
      7. Complete rooming/discharge procedures including: taking and recording vital signs, medical histories and medication records.
      8. Administer injections per protocol and under the direction of a KSM provider if/when applicable. Provide guidance to Medical Assistants on administering injections.
      9. Coordinate off-site procedures, initiate outpatient dialysis placement as ordered by the KSM provider.
    2. Manage Patient Medical Records
      1. Accurately document all information as required by company, state, and federal guidelines.
      2. Maintain patient confidentiality in compliance with HIPAA regulations. 
    3. Manage Clinic Operations
      1. Proactively identify safety concerns, submit incident reports, and discuss with KSM Nurse Manager
      2. Float to other KSM locations based on clinic needs and as directed by Nurse Manager
    4. General Responsibilities
      1. Perform other duties as assigned and proactively identify other tasks needing to be addressed/completed.
      2. Demonstrate professional behavior in interactions with all patients, KSM employees, and affiliated associates.
      3. Provide training and support to other clinical staff around job duties, policies, and procedure.
      4. Participate in nursing and all staff meetings, ongoing in-service trainings, and periodic performance evaluations and development.
    5. Requirements
      1. Current RN licensure with MN Board of Nursing
      2. Current BLS certification
