Job description
Utilizes knowledge and application of nursing principles to provide direct care in the home.
Receives referrals and makes or schedules client visits as needed.
Attend case conference meetings, and takes report on client progress with agency personnel to facilitate coordination of care.
Conducts a minimum of five charts audits per month and reports these findings to the Clinical Supervisor in writing.
Provides services within the scope of practice as defined by At Home Care, Inc
Observes, records and reports to the clinical supervisor the clients response to services and changes in the clients condition.
Schedules home visits for supervisory visit/PCA care for each client monthly
Participates in staff conferences regarding client care.
Participates in all client satisfaction initiatives.
Assist with plan development and implementation for client satisfaction.
