    PROGRAM SUMMARY: Council on Aging (COA) is a dynamic organization dedicated to providing comprehensive senior services throughout Sonoma County. Programs offered include, but are not limited to, Meals on Wheels, PEARLS, Peer Support and Adult and Alzheimer’s Day Programs.

     

    POSITION SUMMARY: Through the combination of marketing and outreach efforts, the Community Engagement Coordinator is responsible for driving the growth of MediCare billed Community Support Services, generating leads for all COA programs, maintaining strong relationships with stakeholders, promoting the organization's mission, and elevating its visibility in the community. With general supervision from the Director of Social and Fiduciary Services and input from other COA Department Directors, this position will develop and implement strategies to expand COA’s network of referral sources and health plan partnerships, as well as represent the organization at events and speaking engagements. Must be flexible to work evenings and weekends as needed to accommodate internal and external marketing events.   

     

    RESPONSIBILITIES:

    Marketing and Outreach

    • Maintain comprehensive knowledge of current COA programs and services to effectively communicate with referral sources, prospects, and community members.
    • Cultivate and maintain relationships with community organizations, healthcare providers, and stakeholders to generate appropriate referrals.
    • Identify and develop new partnership opportunities with health plans, insurance providers, and relevant entities to enhance access to services for underserved seniors and secure new funders.
    • Facilitate direct in-person, telephonic, and written outreach to health plans and other referral sources, conducting interviews with prospects and providers to assess community needs and provide information about COA programs that provide a solution.
    • Develop and manage a network list of community partners and referral sources, ensuring continuing opportunities for business growth and partnerships.
    • Collaborate with COA Directors to achieve program goals and address challenges impacting census, aligning outreach efforts with organizational objectives.
    • Create and execute outreach plans and strategies targeting specific populations to increase awareness of programs and services.
    • Coordinate COA's participation in community events, conferences, and health fairs to promote the organization's mission and engage with potential clients and partners.
    • Organize onsite networking events, educational sessions, workshops, and presentations to educate the community about services and senior-related issues.
    • Manage COA events calendar and share with internal team members to maximize staff participation.
    • Provide input on program marketing materials and manage inventory of promotional items within marketing budgets.
    • Respond to incoming inquiries promptly via phone, email, or virtual meetings, providing accurate information about COA service; assess prospects to determine needs and refer them to appropriate programs internally or externally as needed.
    • Stay updated on trends and best practices in senior services, healthcare, advocacy and community outreach.
    • Maintain comprehensive knowledge of Medi-Cal benefits and accurately share Medi-Cal information when communicating with prospects and referral sources.
    • Participate in business development meetings.
    • Perform other duties as assigned.

    General         

    • Attend scheduled staff meetings and trainings.
    • Respond within one business day to telephone and/or email communications from prospects, clients and community partners.
    • Respond to concerns and complaints in a professional and caring manner; elevate issues to supervisor when appropriate.
    • Submit photos and stories for use in Sonoma Seniors Today publication, social media posts, and fundraising appeals on an on-going basis; ensure photo release is on file for subjects when needed.
    • Track monthly expenses and submit receipts to the supervisor.
    • Maintain required certifications and continuing education hours, if applicable.
