Community Health Director is responsible for developing plans for community-based health programs aimed at the prevention of disease and promotion of health. Recommends community health initiatives, policy implications and best practices after reviewing health literature and statistics. Being a Community Health Director identifies community health program related grant funding and develops grant proposals. Manages specific community health programs from planning and design through implementation and evaluation. Additionally, Community Health Director requires a master's degree in a related area. Typically reports to a top management. The Community Health Director manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. To be a Community Health Director typically requires 5+ years of managerial experience. Deep knowledge of the managed sub-function and solid knowledge of the overall departmental function.     Summary

    The Director of Wound Care oversees and provides acute and rehabilitative care for people with selected disorders of the gastrointestinal, genitourinary, and integumentary systems. The Director also provides direct patient care to people with fistulas, drains, tubes, pressure ulcers, and incontinence in accordance with the philosophy, goals, and objectives of Cozad Community Health System.

    Essential Duties and Responsibilities

    · Utilizes the nursing process for the performance of patient care to assess, plan, intervene, and evaluate patient care to achieve prioritized patient outcomes according to the RN scope of practice.

    · Performs admission assessments, gives medications, performs patient education, and evaluates patient progress towards discharge.

    · Utilizes the nursing process when directing delivery of care to patients identified with wound, ostomy, and incontinence care.

    · Collaborates with Medical Staff, management, and other healthcare team members to coordinate and manage the plan of care for patients.

    · Administers treatments and procedures in a timely and safe manner according to physician orders and nursing policy, using standard precautions.

    · Performs and maintains documentation.

    · Participates in ongoing and systematic quality improvement efforts while providing quality healthcare services to all patients in a financially responsible manner.

    · Ensures adequate documentation and accurate charge capturing for Chargemaster.

    · Develops and maintains the annual operating budget for the Wound Care Department, including productivity and FTE management based on business needs.

    · Maintains licensure/regulatory status for all services.

    · Responds to and resolves patient complaints.

    · Follows all HIPAA guidelines and promotes and ensures patient safety.

    · Fulfills mission statement by performing job duties with a high level of customer service while contributing to a positive and professional environment.

    · Special projects and other duties as assigned.

    Supervisory Responsibilities

    Manages employees in the Wound Care Department. Responsible for the overall direction, coordination, safety and evaluation of this team. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.

    Qualifications

    Education and/or Experience

    Associates Degree in Nursing required. Bachelor’s Degree preferred. Prior wound, ostomy and electronic health records experience preferred.

    Licensure/Certification

    Current Registered Nurse licensure, Pharmacist licensure, Physician Assistant-Certified, or Medical Doctor licensed in the State of Nebraska.

    Basic Life Support (CPR)

    Advanced Cardiac Life Support (ACLS)
