Who are we? NILP is a rapidly growing, human service agency who assists persons with all types of disabilities who wish to live independently in their community throughout Northeast Massachusetts.
NILP provides five core services of Advocacy, Peer Counseling, Information and Referral, Independent Living Skills Training, and Transition.
NILP believes in consumer control and self direction and provides these services with the knowledge that persons with disabilities are the best determinants of their needs and goals.
The role is to serve as a liaison to the enrollee in order to access community based services to improve the quality of life for the enrollee.
We are seeking applicants with experience in service coordination type work with persons with all types of disabilities and elders in the human service field, as well as those passionate about helping others and willingness to show a commitment to Independent Living philosophy.
Applicants must have a strong ability to organize their time between home visits and required paperwork / documentation.
What you will be doing :
Providing counseling on the different options available for long term care, which will vary depending on each consumer's situation
Assisting the consumer in developing a care plan that is person centered and includes services, programs, financial resources, and other supports agreed upon by the consumer.
Providing timely and accurate assistance with referrals to support the consumer in accessing the agreed upon long term supports.
Who we are looking for?
An individual with an Bachelor's Degree in a human service or relevant concentration, can be substituted with personal experience with a disability or equivalent human service experience
Three to five years experience preferred in human services or related field and experience in coordinating services for one or more of the target populations.
Preference will be given for persons living with a disability or with significant personal experience with disability
Bilingual in both English and Spanish is preferred
