Collects patient data to monitor health status and proactively manage the patient's condition. Being a Diabetes Educator facilitates skill development and behavior modification so that the patient is able to successfully manage the disease. Typically requires a bachelor's degree in nursing or equivalent. Additionally, Diabetes Educator typically required to be a Certified Care and Education Specialist (CDCES).     Job Summary

    The Diabetes Care and Education Specialist (DCES) delivers comprehensive and seamless services that bridges the gap to integrate clinical and self-management aspects of diabetes and cardiometabolic care in all settings (acute, ambulatory and community). The DCES is an integral part of the interprofessional team and provides collaborative, comprehensive and person-centered care and education conducive to behavior change and improved quality of life across the lifespan. The DCES supports and advocates for people affected by diabetes to optimize quality care. The DCES promotes self-management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes. The DCES is a team member of a recognized American Diabetes Association (ADA) Diabetes Self-Management Education and Support (DSMES) department and follows the ADA recognition requirements which are based on the National Standards of DSMES guidelines.

    Accountabilities

    • Integrates skills and knowledge of pathophysiology, epidemiology, clinical management, cardiometabolic conditions, and self-management of diabetes into clinical practice. 10%

    • Advocates for and communicates about improved quality of care and outcomes for those living with, at risk for, and affected by diabetes and cardiometabolic conditions. 10%

    • Partners with individuals to deliver care and education conducive to behavior change and improves quality of life by applying the Association of Diabetes Care and Education Specialist (ADCES) ADCES7 self-care behaviors for self-management of diabetes and cardiometabolic conditions across the lifespan. 10%

    • Applies business principles, systems practice, and population health management to support achievement of the Quintuple Aim (reduced costs, improved outcomes, improved experience, improved work life for healthcare providers, and equity in health and social services). 10%

    • Contributes to the achievement of established DSMES department goals and objectives and adheres to department policies, procedures, quality standards and safety standards.\u00A0 Complies with governmental and accreditation regulations. 10%

    • Provides quality diabetes self-management education and medical nutrition therapy (limited to Registered Dietitians) in individual and group settings based on assessed needs.\u00A0 Utilizes appropriate teaching techniques that are sensitive to the learning preferences of the person with prediabetes or diabetes. 15%

    • Completes comprehensive assessments for each patient including emotional and behavioral health, interprets personal health data, develops an individualized care plan based on the patient's assessed needs and goals and promotes self-management. 10%

    • Collaborates, advocates, and confers with other members of the diabetes care team in developing person-centered diabetes plans.\u00A0 Engages in lifelong learning and serves as a role model of professionalism. 10%

    • Advocates for and supports technology-enabled diabetes education and care, in individual and population health diabetes services. 15%

    Minimum Requirements

    Bachelor's, Master's or Doctorate degree in\u00A0 dietetics/nutrition, health education, pharmacy, physical therapy, occupational therapy, clinical psychology, optometry, podiatry, exercise physiology, physician assistant or Master's degree in social work.

    1 year of Clinical experience. Experience with diabetic patient population preferred.

    Experience with diabetic patient population preferred.

    Required Licensure, Certification or Registration

    Licensed to practice in South Carolina as one of the following: Registered Dietitian, Physical Therapist, Occupational Therapist, Pharmacist, Physician Assistant, Optometrist, Podiatrist, Clinical Psychologist, LMSW or LISW.

    Will also accept Exercise Physiologist holding certification as an ACSMC Clinical Exercise Physiologist or Health Educator holding certification as Master Certified Health Education Specialist from the NCHEC.

    Job related certification from one of the following:

    Certified Diabetes Care and Education Specialist (CDCES) from the Certification Board for Diabetes Care & Education (CBDCE); or

    Board Certified-Advanced Diabetes Management (BC-ADM) from the Association of Diabetes Care & Education Specialists (ADCES).

    Other Required Skills and Experience

    Knowledge of diabetes technology including insulin pumps and continuous glucose monitors preferred.

    In Lieu of Statement

    In lieu of certification, will consider candidates that are certification eligible and willing to obtain required CDCES or BC-ADM certification within three (3) years of DCES hire date.\u00A0 Additionally, must complete 15 CEUs in diabetes-related education within 4 months of hire and each ADA anniversary year until certification is obtained. \u00A0
