The Care Management Team at Midwest performs duties for the full life cycle of clients, ranging from care planning for special needs children to geriatric care management and end-of-life planning. Duties include conducting assessments, care planning, advocacy and acting as a liaison between involved parties. We are professional, agile, and supportive.
