LCCA currently has an exciting opportunity for a Discharge Planner. The Discharge Planner is responsible for
- coordinating the discharge plan offered to the patient
-
interfacing with the treatment team/managed care organizations to ensure quality patient care,
- collaborating with next level professionals for continuity of care,
- obtaining certification for services for next level of care/ providers.

functioning as a member of the multidisciplinary team and participate in the community relations program of the organization

participating in continuous performance improvement activities
-
communicating with outpatient referral sources, families/guardians, and outpatient aftercare providers.

Qualifications:
Regulatory Requirements

Masters Degree in a behavioral science related field (psychology, social work, human services, or counseling)
Language Skills:

Ability to communicate in English, both verbally and in writing.

Must have sufficient language skills and ability to communicate both verbally and in writing to accurately and professionally implement and document the services provided to clients.

Additional languages preferred.
Skills:

Basic computer knowledge.

Strong interpersonal skills.

Knowledge of a wide range of psychosocial, cognitive and /or developmental disorders.

Ability to visually observe client needs.

Possess the ability to work productively and effectively.
Physical Demands:

Bending, stooping, lifting and pulling during direct client care necessary as required. May be required to assume awkward positions during physical management of violent or emotionally disturbed clients. Ability to safely participate in and/or supervise physical leisure activities of designated client population.
WORKING CONDITIONS:

Generally works in a fluorescent lit, enclosed, non-smoking environment. Occasionally required to supervise clients who wish to spend time outdoors on a daily, year-round basis. Possible job related hazards include lacerations and punctures from contaminated needles or sharp objects, infectious diseases, skin and eye irritations from cleaning agents, and human bites, scratches and/or saliva.
