Audits medical records to guarantee compliance with institution standards, procedures and policies. Being a Discharge Coordinator may require a bachelor's degree in area of specialty. Education/Licensure: Bachelor’s Degree from an accredited college or university, in Social Work or a clinical related mental health field.
Experience: Previous experience in a psychiatric health care facility, with direct experience in case management, care coordination and interdisciplinary team staffing preferred.
Additional Requirements: Required to work flexible hours and weekends.
