A prestigious hospital in Southern New Hampshire is searching for an Admission and Discharge Nurse to join its growing team! In this role, you will provide professional nursing care to patients, including assessment, treatment, care planning, and medical care evaluation. Responsibilities:

• Deliver direct and indirect patient care using the nursing process.
• Assess patients upon admission to the ED
• Obtain and document health history, vital signs, and health risk factors.
• Plan and implement patient care plans.
• Administer medications and document patients’ progress toward desired outcomes.
• Assist with discharge planning.
• Collaborate with interdisciplinary teams to improve patient care processes and outcomes.

Qualifications:

• An ASN is required, and a BSN is preferred.
• Active RN license.
• 2 years of clinical nursing experience is preferred.

Additional info:

• Permanent position, 32 hours/week
• Schedule; 4x8, 9a-5p
• NO WEEKENDS OR HOLIDAYS!
• Pay; $35-45/hr., depending on experience, differentials.
