Responisble for coordianting discharges at two of our sister facilities.
Responsible to assist Social Service Coordinator and Social Wrokers in providing medically related social services so that each resident /patient may attain or maintain the hightest practicable level of physical mental and pshychosoical well-being.
Assists in implementing the resident/patient's discharge/patients and families
Meets with resident as needed to provide information and facilitate adjustment
Performs any miscellaneous work assignments as may be required.
