Work Schedule:
- Shifts: Sunday - Saturday
- Shift Durations: Various (4, 8, 10, or 12 hours) on rotating basis
- Additional Work Requirements: On-Call, Call Back, and Overtime may be required
Minimum Qualifications (Applicants must meet the following requirements):
Education and Degree:
- A Bachelor of Science in Nursing (BSN) degree from an accredited nursing program recognized by the US Department of Education.
Experience:
- Minimum of 5 years of nursing experience.
- At least 36 months of recent experience in direct patient care in a hospital or acute care discharge planning setting.
- Extensive knowledge of nursing care principles and procedures, TRICARE concepts, and clinical training.
- Familiarity with discharge planning and understanding of "safe discharge."
- Knowledge of Interqual/Milliman and evidence-based guidelines.
- Understanding of relevant laws like EMTALA and COBRA.
- Current unrestricted RN license in Nevada.
- Knowledge of TRICARE, Medicare, Medicaid, and private health insurance policies.
- Understanding of DRGs, ICD-9, CPT-4, HCPCS coding, and LOS criteria.
- Knowledge of medical facility organization and communication skills.
- Ability to evaluate treatment plans and use computer applications/software for data management.
Licensure:
- Possession of a current unrestricted Registered Nurse license in any state.
Clinical Certification (Preferred):
- Certification in a relevant specialty, such as Certified Managed Care Nurse or Certified Informatics Nursing.
- Other relevant certifications like Ambulatory Care Nursing, Medical-Surgical Nursing, or Nursing Case Management from the American Nurses Credentialing Center.
Life Support Certifications:
- Possession of current ARC or AHA BLS, PALS, and ACLS certifications.
Security:
- Must pass a Government background check.
- Must obtain a Government security clearance.
Core Duties:
Job Specific Duties:
- Provide discharge planning services for all inpatients and same-day surgery outpatients at the medical treatment facility (MTF).
- Efficiently triage discharge planning needs, creating templates for routine processes to improve service delivery.
- Predict discharge/transfer needs accurately to prevent unnecessary delays or extended Lengths of Stay (LOS).
- Ensure smooth transitions from inpatient care to the appropriate level of care, involving patients, families, caregivers, and the healthcare team.
- Collaborate with utilization management, case management, and other healthcare team members to achieve timely and safe discharges in compliance with local, Air Force (AF), and Department of Defense (DoD) standards.
- Use evidence-based programs like Interqual (McKesson)/Milliman to monitor Length of Stay (LOS) and admission/discharge criteria.
- Assist in arranging patient transfers to other facilities and obtaining necessary authorizations and transport arrangements.
- Monitor care quality across the continuum of care and report concerns to relevant parties.
- Participate in committee meetings, working groups, and other relevant groups as needed.
- Coordinate with clinical units for discharge planning requirements, including Durable Medical Equipment (DME) and referrals.
- Monitor LOS trends and identify processes affecting extended stays.
- Coordinate with third-party payers and TRICARE Managed Care Support Contractor (MCSC) for authorization and referrals.
- Provide education to patients, providers, and staff on discharge planning and inpatient transfers.
- Ensure patients receive instructions for post-discharge care and education beyond insurance benefits.
- Educate medical students, residents, and other personnel on discharge planning and transfer processes.
- Participate in orientation and training of new personnel.
- Maintain positive relationships with colleagues, families, peers, and healthcare team members.
- Effectively communicate and collaborate with diverse groups for the benefit of patients and the organization.
- Maintain accurate documentation in accordance with requirements.
- Contribute to a professional environment using appropriate leadership skills.
- May work various shifts or provide clinical nursing support during the absence of team nurses.
- Ensure a safe work environment, infection control compliance, and patient safety.
