Hospice Supervisor manages the resources, operations, and staff of hospice care programs. Works with families and patients to develop optimal care plans. Being a Hospice Supervisor coordinates the delivery of a patient/family assessment process and care plan development. Oversees interdisciplinary teams of medical doctors, nursing staff, and other care providers inside and outside the facility. Additionally, Hospice Supervisor maintains an up-to-date knowledge of laws and regulations of federal, state, and regulatory bodies relative to hospice care. May require a bachelor's degree in nursing, social work or equivalent. Typically reports to a manager or head of a unit/department. The Hospice Supervisor manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions. To be a Hospice Supervisor typically requires 5 years experience in the related area as an individual contributor. 1 - 3 years supervisory experience may be required. This expansion presents a unique opportunity for a seasoned hospice RN to lead a compassionate team as the Hospice Clinical Supervisor at our Waverly office.     DUTIES OF POSITION

    The Clinical Supervisor is responsible for ensuring the coordination and management of patient care. Assists the Director of Clinical Services in maintaining the quality of patient care in accordance with the Agency’s objectives and policies, through planning, coordination, implementation and evaluation of the hospice services provided.

    POSITION RESPONSIBILITES

    Coordinate and oversee direct and indirect patient services provided by clinical personnel.

    Provide supervision and guidance to clinical staff to ensure the quality of services and maintenance of standards.

    Perform chart audits.

    Gather data for QAPI and participates in QAPI activities.

    Identify and evaluate patient needs and appropriateness of home care by reviewing the initial intake screening form before assignment of direct care staff.

    Assist with the orientation of new staff.

    See patients in the home setting as needed.

    Maintains adequate and appropriate Medical Supplies.

    Assists with orientation of new personnel as well as in-services for current staff.

    Responsible for ensuring Medicare, State and ACHC regulations are followed by all clinical staff.

    Assists the Director of Clinical Services with responsibilities as requested.

    Appraise clinical staff performance during their probationary period and annually, provides the teaching and counseling necessary to enhance the delivery of services and the professional development of nursing and health aide staff.

    Assume responsibility for self-development by continually striving to improve his/her Nursing/management practice through formal education, attendance at workshops and conferences, active participation in professional and related organizational meetings, and/or individual research and reading.

    JOB CONDITIONS

    Position may be stressful.

    Primarily a desk job which involves minimal and occasional lifting of medical supplies and charts.

    On occasion, one may be required to bend, stoop, reach and move patient weight up to 250 pounds lift and/or carry up to 30 pounds.

    Travel is required to and from patient's home when supervision of care is scheduled.

    The ability to access patients’ homes which may not be routinely wheelchair accessible is required.

    Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform patient care.
    EQUIPMENT OPERATION

    Use of computer, multi- line telephone, calculator, copier, and other office and medical equipment.

    COMPANY INFORMATION

    Has access to all patient medical records and patient financial accounts as well as personnel records of department staff.

    QUALIFICATIONS

    1. Must be a graduate of an accredited School of Nursing.

    2. Must be a Registered Nurse licensed by the state of OHIO. Must have no exceptions to RN license, and be able to administer pharmaceuticals, perform medication reconciliation, and any nursing procedures or special treatments needed within the scope of nursing practice for state

    3. Must have a minimum of 2 years nursing experience in home hospice or skilled home care. (hospice preferred)

    4. Ability to market and deal tactfully with customers and the community.

    5. Knowledge of business management, governmental regulations and ACHC standards.

    6. The ability to communicate well, both verbally and in writing.

    7. Must have the ability to demonstrate self-confidence and positive attitude toward self and others and maintain commitment and enthusiasm to goal achievement.
