Healthy Living Intensive Case Manager
Hours: Monday - Thursday
Status: Full-Time
Reports to: Adult Housing Programs Supervisor
Purpose:
Under the direct supervision of the AHP Supervisor, this position provides direct case management services to medically fragile, chronically homeless adults.
Duties, Functions, and Responsibilities:

Conduct initial interview/intake and clients' needs assessments.

Provide support and arrange services to consumers moving into long-term housing, to promote a transition from homelessness to residential stability.

Effectively explain public social services programs and assist consumers with the application and follow-up to ensure services are received.

Create consumer-driven treatment plans in concert with consumer and doctor recommendations that reflect the needs of the medically fragile client.

Monitor the client's progress and provide follow-up services.

Maintain accurate documentation of service to clients. Enter accurate consumer data into HMIS in real-time data, to reflect consumer activities and their progress.

Maintain audit-ready files at all times. (Random audit checks are regularly performed).

Participate in multi-agency, multi-disciplinary teams to report and review consumer progress.

Other duties, as may be assigned.
Knowledge, Skills, and Abilities:

Knowledge of local social service agencies and programs as they pertain to the local homeless population.

Knowledge of applicable statutes, ordinances, and codes.

Knowledge of social service work, and interviewing/referral techniques.

Experience and understanding of homeless consumer issues.

Understand and follow complex instructions and procedures. Analyze situations accurately and adopt an effective course of action.

Communicate effectively with persons from a variety of social, cultural, and economic populations.

Effective written, verbal, and listening skills. Independently initiates and responds to correspondence.

Ability to maintain order and exercise appropriate judgment in crisis situations.

Is flexible, and can quickly adjust to shifting priorities.

Establish and maintain cooperative working relationships with consumers, fellow employees, other agencies, and the general public.

Organize work schedule and budget time.

Understand and follow all agency policies and procedures.

General computer knowledge, and the ability to use Microsoft Office software.

Ability to work flexible hours, as required by consumer caseload.

Ability to perform accurate mathematical equations to calculate client's rent and utility costs, and request funding to pay those bills.
Qualifications, Education, and/or Equivalent Experience:

Bachelor's Degree in Social Work, or High School Diploma and (2) two years of experience, substantially equivalent to a bachelor's degree, or combination of education and experience.

Professional experience, as required by education.

Must pass a background check and pre-employment drug screening. Random drug testing is performed.

Must have a valid NV driver's license and a clean DMV record. Driver History Reports are required to be submitted on a quarterly basis.
