Crescent Home Health Agency is looking for a dynamic person to join our team as Intake Coordinator. This person must have a positive attitude, excellent customer service and organizational skills, and the ability to multi-task. The ideal candidate cares about patients and ensures they receive the care they need. This role will cover all aspects of coordination for new as well as resuming patients (from entering referral information to scheduling clinicians to ordering supplies).
Required Qualifications:
Prior experience in intake coordination (preferably in a home health setting), knowledge of medical terminology and the ability to interpret information accurately.
