Nye Health Services is seeking an experienced clinical professional to join our home healthcare team as Intake Coordinator. This centralized role will lead the intake and admissions process for Nye Home Healthcare. As Intake Coordinator, you will review referrals and work with clinical and therapy teams to evaluate potential client’s medical needs before admission, serve as a resource for referring hospitals by developing relationships with their physicians and case managers to achieve census goals and identify potential clients that will benefit from Nye Home Healthcare quality care and services.
This role is primarily administrative and could offer flexibility in work location including the possibility of working remotely on a hybrid basis. Candidates should be willing to live within a reasonable driving distance of Fremont, NE as routine office time will be necessary.

In this role you will:
- Coordinate patient referrals and prepare admission documentation for Nye Home Health Care.
- Develop and maintain an excellent working relationship with referral sources and medical professionals who refer to Nye Home Health Care.
- Interview and evaluate patients as appropriate to determine if they are suitable for admission to the home health agency.
- Develop an understanding of payor sources and complete pre-authorization tasks to obtain insurance coverage, based on the payor source.
- Routinely attend case management meetings/hospital rounds and/or SNF discharge planning meetings to identify and discuss potential referrals.
- Communicate new referral information to the home health team to ensure a smooth transition to home with home health services initiated.
- Provide assistance in preparation for, and during, the state, federal, and ACHC survey processes.
- Participate and attend regularly scheduled meetings to benefit the team, including, but not limited to QAPI, team stand-up meetings, case conferences, safety committee, and other pertinent meetings as directed.
- Maintain knowledge of the agency’s referral processes to ensure that pertinent clinical and financial information is gathered and entered into the EMR for each new referral.
- Maintain, update, and access the current referral resource base, keeping apprised of trends and developments in referrals.
- Communicate with the Director of Nursing / Clinical Coordinator regularly regarding patients’ clinical needs, scheduling/staffing capability, or regulatory issues.
Qualified Candidates:
- RN or LPN licensure preferred, or prior experience that will create success in the role.
- Prior home health or admissions/case management experience preferred.
