Hours: Full-Time with occasional weekends

Salary: $24 to $28 per hour

Intake Coordinator Responsibilities:

• Efficient Referral Processing: Coordinate and process referrals for home health care services from various sources.
• Patient Assessment: Conduct initial screenings to determine patient eligibility for our services.
• Thorough Documentation: Obtain all necessary information from patients and their families and accurately document patient care plans in our system.
• Effective Communication: Keep all stakeholders informed of patient care plans and any changes that may occur.
• Collaborative Approach: Work closely with clinical and billing departments to ensure seamless patient care and billing processes.
• Record Maintenance: Maintain accurate and organized records of patient information and care plans.

Intake Coordinator Qualifications:

• Experience: Minimum of 1 year of experience in a healthcare setting, preferably in home health care or a related field.
• Education: High school diploma or equivalent (Associate's or Bachelor's degree in healthcare or related field preferred).
• Skills: Strong organizational skills, excellent communication skills (both written and verbal), proficiency in Microsoft Office and electronic health record (EHR) systems.
• Flexibility: Availability to work flexible hours as needed, including evenings and weekends.
