The Courville at Nashua is a Senior Healthcare Community in Nashua, NH. We are locally owned and operated and committed to providing personalized service and care. We’re looking for Full-time MDS/Managed Care Documentation Specialist (RN preferred) to join our dedicated staff.

Job Description: This position is perfect for an experienced healthcare professional. Successful candidates will have a passion for human kindness and wellness for life. This position may offer some flexibility, but it is primarily a site-based position and requires coming into the office.

Case Manager responsibilities include:

• Update insurance companies on patients’ clinical status
• Submit patient health information to agencies for appeal processes
• Ability to navigate through electronic medical records
• Strong organizational skills to manage case mix and deadlines
• Strong communication skills to inform managers of patient’s status
• Customer service driven when assisting patients with insurance coverage

MDS Coordinator:

• Complete ICF & PPS MDS’ in a timely manner
• RAPS
• Comprehensive Care Plans on ICF & Skilled Residents
• Update care plans with quarterly’s 30 days and 60 day Medicare assessments
• Provide care plan schedule to Social Worker in timely manner
• Schedule weekly assessments with Recreation Director, Social Worker, Dietician & Rehab Director
• Complete overall Quarterly and Annual schedule of MDS assessments for ICF residents
• Complete monthly schedule of ICF MDS assessments due for Unit managers on 1st floor 2nd and 3rd floor
• Able to complete assessments utilizing MDS 3.0 according to long term care regulations 
• Proficient with computer skills 
• MDS 3.0 experience 
• Knowledge of Point Click Care systems
