Medical Records Coding Technician abstracts clinical information from medical records and assigns the appropriate ICD or CPT codes using industry-standard coding guidelines. Assigns required DRG (diagnosis-related grouping) codes. Being a Medical Records Coding Technician works with coding databases and software to input and maintain data according to standard procedures. Performs quality audits of work. Additionally, Medical Records Coding Technician maintains and up-to-date knowledge of coding and documentation requirements. Requires a medical coding certification. The exact type of coding certification may vary based on the clinical setting or a medical specialty focus. May require an associate degree. The AAPC Certified Professional Coder (CPC) certification is typically required. The Certified Coding Specialist (CCS) certification is also a typical requirement. Typically reports to a supervisor or manager. The Medical Records Coding Technician works independently within established procedures associated with the specific job function.     Coding Specialist Sr. Medical Records 

     

    Utilizing 3M encoding system, the Senior Coding Specialist is responsible for accurate assignment of I-10 and CPT codes to inpatient and outpatient medical records for the purpose of reimbursement, research, and compliance with federal regulations.  Coding Staff are required to meet coding quality and quantity standards.  Also Coding Staff are responsible for researching newly identified diagnoses and/or procedures for code assignments. They also perform all other duties as assigned by Manager or Lead Coder.

    Responsibilities

    • Utilizes the 3M encoder system to assign I-10 and CPT codes to inpatient medical records and calculate DRGS.             
    • Codes clinic, emergency room, and other outpatient records utilizing I-10 and CPT coding system.
    • Codes all cycle bills for Patient Accounts so that an interim bill can be generated.
    • Consistently codes 22 inpatient or 35-40 outpatient surgery/OBS records per day and 300 diagnostic accounts per day..      
    • Consistently codes at 95% accuracy rate or better (minimum quality standard). 
    • Meet weekly coding deadlines to bill within NCH hospital suspense of 6 days.
    • Works successfully in a team environment to achieve coding goals and deadlines.
    • Utilizes Internet and other resources to research newly identified diagnoses and/or procedures.
    • Assists physicians in research projects by identifying the necessary ranges of disease or procedure codes.
    • Researches billing issues for Patient Accounts by analyzing edits and account denials.
    • Exhibits excellent customer service skills in daily contact with internal and external customers.
    • Identifies process problems in daily workflow and takes the initiative to solve them.
    • Uses Streamline Health, Provation, Cardio IMS, and Epic to access documentation for code assignments.
    • Reviews HIM workqueues for unbilled accts out of deadline and completes coding.
    • Performs inpatient and outpatient coding audits at direction of Coding and Compliance Manager.
    • Participates in Coding In-service programs and outside educational meetings.
    • Completes required on-line coding education modules as assigned.
    • Accesses CDI software to populate coding APRDRG and evaluate variances.
    • Educate CDS Specialists on I-10CM coding.
    •  Communicate with CDS team using CDS Software, phone or in person regarding APRDRG mismatches.

    • Query providers to confirm diagnoses, procedures or specificity.
    • Performs other duties as assigned by the Coding and compliance Manager.

    Qualifications

    KNOWLEDGE , SKILLS AND ABILITIES REQUIRED

    • Health Information technology program graduate with successful completion of accreditation examination preferred.
    • CPC, COC or CCS certification required if RHIT certification not completed.
    • Minimum 2-3 years of acute care coding experience preferred.
    • Working knowledge of ICD-10CM and I-10 PCS coding, CPT, and DRG’S.
    • Epic familiarity preferred.
    • Computer literacy required with ability to key entry into 3M quickly.
    • Experience with 3M encoder system preferred
    • Ability to handle multiple tasks, priorities and deadlines.
    • Analytical and organizational skills a must for analyzing charts and data.
    • Effective communication and interpersonal skills required with ability to work with physicians and other caregivers regarding coding issues.

    MINIMUM PHYSICAL REQUIREMENTS

    Must be able to see, hear, speak, read, and perform manual tasks with or without accommodation and care for oneself, with little or no difficulty. Must be able to sit for long periods of time and be able to key entry for the duration of daily shift.
