Medical Social Worker (BSW) jobs in Vermont

We are looking for a Medical Social Worker (MSW) to support hospice patients at home. In this role you will collaborate with members of our interdisciplinary teams to ensure that your clients’ non-medical needs are met and that they have the supports that they need in place.

This role is eligible for a $8000 sign-on bonus

Primary Job Responsibilities

• Working in hospice, you will have the unique opportunity to provide direct, one-on-one supports to individuals in a setting where they are most comfortable, at home
• Develop trusting, meaningful relationships with your clients over extended periods of time – something that is not possible in a hospital or facility setting
• Develop, communicate, and coordinate a goal-oriented plan of care in collaboration with the patient, their medical doctor, and family. This includes care plans for LNAs as needed
• Maintain consistent, timely, and accurate communication with all members of the patient’s health care team
• Keep up to date electronic documentation of care provided
• Evaluate each person’s progress in meeting their goals of care and update plan of care accordingly
• Evaluate evidence-based treatments for clients with psychosocial challenges that interfere in their ability to abide by medical plan of care

Job Requirements

• Master’s degree in social work or Master's degree with Vermont state licensure in mental health or marriage and family counseling
• Minimum one year of progressive clinical experience in medically-oriented setting.
• Exceptional new graduates will be considered
