Medical Social Worker (MSW) provides social work services to hospital patients and their families. Counsels and provides crisis intervention for patients, including assessment and treatment of emotional and behavioral problems related to hospitalization. Being a Medical Social Worker (MSW) participates as a member of multi-disciplinary team and may assist patients in treatment planning. May contribute to the formulation of discharge plans. Additionally, Medical Social Worker (MSW) may take a leadership role on more complex cases. Requires a master's degree of social work. Typically reports to a manager. As a Medical Social Worker, you will support patients, families, and caregivers in their homes, helping them to cope with social and emotional issues and making practical arrangements relating to end-of-life. You will serve as an integral member of the healthcare team, providing assessments, coordination, treatment planning, information, and referral to community resources and other social work services to meet the complex needs of every patient and family.
Qualifications
- Master’s of Social Work (MSW) and licensure or Bachelor of Social Work and Licensure (Must be licensed in the State of New Mexico)
- Exceptional communication skills (multilingual is a plus )
- Exceptional professionalism
- Team-care oriented
- Experience working in geriatrics, hospice, or end-of-life care
- Valid driver’s license
