Minimum Data Set (MDS) Coordinator (LPN) assesses long-term care patient health needs in collaboration with the Minimum Data Set (MDS) Coordinator (RN). Assists in developing a comprehensive care plan. Being a Minimum Data Set (MDS) Coordinator (LPN) maintains medical records and ensures documentation is present for coding. Adheres to federal, state, and local regulations and organizational policies and procedures. Additionally, Minimum Data Set (MDS) Coordinator (LPN) requires a high school diploma. Requires Cardiopulmonary Resuscitation (CPR) and Licensed Practical Nurse (LPN) certification. Typically reports to a mds rn. Job Overview:
· Utilizes MDS Scheduler to plan and set MDS Assessment Reference Dates (ARDs).
· Communicates ARD selection for specific MDSs to IDT.
· Completes the MDS sections as assigned.
· Completes CAAs as required.
· Completes Care Plan items and attends Care Plan meetings as assigned.
· Monitors overall the documentation in the medical record to validate that it supports MDS coding.
· Participates in skilled utilization and State Case Mix management as assigned.
· Provides teaching and training for MDS item completion to interdisciplinary team members that have responsibility for MDS item completion.
Qualifications:
· Must possess a current, unencumbered, active license to practice as an RN Nebraska, or have an active compact license.
· Experiencing with RAI process and MDS certification preferred.
