Job Summary:
We are seeking a skilled and compassionate Nurse Practitioner to join our healthcare team in the Salt Lake County and Utah County Areas. As a Nurse Practitioner, you will provide high-quality care and support to patients across various settings in their homes or Assisted Living resident. Your expertise in geriatrics, primary care, and medication management will be essential in delivering comprehensive healthcare services.

Responsibilities:
- Conduct thorough patient assessments and develop individualized care plans
- Perform diagnostic tests and interpret results
- Administer medications and treatments
- Monitor patient progress and make necessary adjustments to treatment plans
- Educate patients and their families on health management strategies
- Collaborate with other healthcare professionals to ensure coordinated care
- Provide guidance and support to nursing staff

Qualifications:
- Valid Nurse Practitioner license in [state]

Pay: $111,299.68 - $134,038.33 per year
