The NH Care Collaborative CFI Program is seeking a energetic, positive, compassionate professional to join their teams.
The Choices for Independence (CFI) Care Coordinator position is responsible for coordination of services for clients. They complete assessments, develop a care plan in conjunction with the client, coordinates services identified and documents and monitors services provided. Work is performed with minimal supervision and a high degree of self-initiative and independence in keeping with the standards of excellence set forth by the program. Care Coordinators utilize their home office and are required to travel for client in-home visits. Travel will be within one hour of staff home. Clients are in the area of Manchester, Nashua, Milford, Hillsborough, Concord, etc.
Position has a great deal of flexibility and independence. Terrific time off as well!
Have demonstrated knowledge of the local service delivery system and the resources available to participants.
Have demonstrated knowledge of the development and provision of integrated, person-centered services.
Bachelor’s Degree preferred, 4 years relevant experience, driver’s license and reliable transportation. Multilingual helpful.
