Seeking a certified Case Manager for our behavioral Health Outpatient Clinic Program specializing in Dual Diagnosis, Alcohol and or Substance Abuse. The individual must be willing to work with individuals having a wide variety of mental health symptoms and behaviors.

The Case Manager provides ongoing professional, technical and clinical expertise/support through comprehensive assessment, planning and treatment, with a focus on coordination of care and individual patient needs. Case Managers are a vital part of the treatment team whose overall objective is to enhance the quality of patient management, treatment and satisfaction. This goal is achieved by facilitating a smooth and efficient continuity of care for patients, helping the patient maximize their treatment.
They must be computer literate, proficient in MS Office including Word, Outlook and Excel. Strong communication skills in the form of both written and verbal.
Duties and Responsibilities:
1. Serves as primary point of contact for patients to coordinate their treatment while at our clinic, including both internal and external appointments and support needs.
2. Provides support for the Clinical Treatment Team by facilitating groups due to overflow and/or other clinical needs, as determined by the clinical behavioral director.
3. Maintains active files and networks for community referrals.
4. Serves as a clinical point of contact for patient families during the course of treatment at our clinic.
5. Communicates with patients’ families at a frequency of at least one time per week (as appropriate based on existing Releases of Information (ROIs)) in order to update families on treatment and discharge planning.
6. Coordinates initial and follow-up medication appointments with medical assistants.
7. Provides referrals and assistance in filling out forms for disability and other community and social support programs.
8. Meets face to face with each individual patient on Case Manager’s case load for at least 45 minutes per week to review treatment, discharge plans and other patient and clinical concerns.
9. Attends Clinical Staffing meeting each week and be actively involved in the case review and discussion process.
10. Maintains regular charting and notes in the electronic medical records system on patient meetings and case management efforts (including documenting any emails sent and any phone calls made for the client for appointments or updates).
11. Coordinates and implements discharge planning, with involvement/input from the Clinical Team, patient and patient’s family (as applicable), whilst considering the personal and clinical needs of the patient.
12. Coordinates discharge plans with the Alumni program as a component of follow-up care.
13. Collaborates with Intake/Providers and Administration and assists with discharge referrals.
14. Participates in ongoing trainings, supervision meetings, and emergency treatment.
15. Other duties will be assigned if need be by operations for any overflow.
