Pharmacist - Home Care compounds and dispenses prescribed medications and pharmaceutical supplies per physician's instructions for patients in a home care setting. Provides consultation and answers inquires from patients, families, physicians and other healthcare professionals regarding drugs and their use. Being a Pharmacist - Home Care maintains all required documentation and data on prescriptions, inventory, and patients. Adheres to JCAHO standards for Home Care and complies with all regulatory guidelines and procedures. Additionally, Pharmacist - Home Care requires a doctorate degree in pharmacy (Pharm.D.). Typically reports to a manager or supervisor. Requires Registered Pharmacist (RPh). Pharmacist - Home Care's years of experience requirement may be unspecified. Duties
- Provide compassionate and personalized care to individuals in their homes or in a nursing home setting.
- Assist with activities of daily living, including bathing, dressing, grooming, and toileting.
- Medication reminders
- Monitor and record vital signs, such as blood pressure and temperature.
- Assist with mobility and transfers, including using assistive devices and lifting individuals when necessary.
- Provide companionship and emotional support to clients.
- Perform light housekeeping tasks, such as cleaning and laundry.
- Prepare meals and assist with feeding if needed.
Qualifications
- Previous experience in caregiving or a related field is preferred.
- Knowledge of caregiving systems and techniques.
- Ability to provide patient care with compassion and empathy.
- Strong communication skills to effectively interact with clients, their families, and healthcare professionals.
- Ability to perform heavy lifting and physical tasks as required.
- Experience in senior care or childcare is beneficial.
- Must be reliable, punctual, and able to work independently.
