Pharmacist - Home Care compounds and dispenses prescribed medications and pharmaceutical supplies per physician's instructions for patients in a home care setting. Provides consultation and answers inquires from patients, families, physicians and other healthcare professionals regarding drugs and their use. Being a Pharmacist - Home Care maintains all required documentation and data on prescriptions, inventory, and patients. Adheres to JCAHO standards for Home Care and complies with all regulatory guidelines and procedures. Additionally, Pharmacist - Home Care requires a doctorate degree in pharmacy (Pharm.D.). Typically reports to a manager or supervisor. Requires Registered Pharmacist (RPh). Pharmacist - Home Care's years of experience requirement may be unspecified. As a Lead Caregiver at Trail Ridge Home Care, you'll help to orientate new team members, provide mentorship and training to the caregiver team as requested, and assist in filling in client assignments and on-call rotation as needed.
You'll also work in a caregiver capacity, providing companionship and assistance with personal care. You'll work directly with seniors in their homes using a variety of techniques to encourage and support their ability to maintain or attain their highest level of physical, mental, and social well-being. You'll take pride in managing the needs and care regimen of your clients and partner closely with our courteous business office staff to develop care plans.
