What you can expect:
A Case Manager, Generalist provides moderately to highly complex, direct, client-centered case management services and care to patients while seamlessly linking them to medical care and other supportive services at Howard Brown Health. A Case Manager, Generalist is responsible for working as a part of the Community and Social Health Services, as part of the PCMH model, through screenings and assessments of need, service planning, linkages and referrals to public and private services and programs, information dissemination, advocacy, mental health assessment, service plan follow-up, and discharge planning.
How you will make an impact:
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Manages a caseload of assigned HBH participants with support.
- Coordinates and assists with assigned HBH participant intakes, completing PHQ-9 questionnaires, and needs assessments.
- Assists participants in developing service plans.
- Provides ongoing assessments and evaluates the effectiveness of service plans and progress made by participants and participates in client treatment planning and case review with the manager of program staff.
- Coordinates and conducts a comprehensive assessment process for assigned participants, as appropriate.
- Develops and monitors a regularly reviewed action plan in partnership with the participants and Social Service program team.
- Provides case management services including monitoring participant needs and making referrals as needed, in consultation with the Social Services program team.
- Identifies and coordinates resources for direct medical, clinical, and psychiatric care of program participants with program staff.
- Ensures all parties are kept informed about the medical and psychiatric needs of the participants and completes referrals in a timely manner.
- Coordinates with program staff to ensure that participants are active and engaged in services, appointments, meetings, and activities.
- Contacts enrolled participants monthly by phone, email, video, or in person for individual case management.
- Documents and updates all interactions with enrolled and new participants, writes clear and concise notes, and maintains daily logs on all participants.
- Maintains and reports applicable statistics regarding programs and participant services.
- Manages any crisis and/or emergencies that may occur on the units with patients.
- Provides telephone and/or in-person screening to patients that are requesting immediate service.
- Responds to all shift emergencies and effectively completes all documentation.
- Prepares and accurately submits all required forms and statistical reports, maintains documents regarding participant’s cooperation, daily activities, and other records in compliance with agency and external policies, procedures, and applicable licensing standards.
- Participates in and contributes to regularly scheduled supervisory and team meetings.
- Participates in program planning and evaluation.
- Performs related duties as assigned.

Case Manager I
Education and Experience:
- High school diploma/GED or equivalent is required and 4-6 years of experience working in a community based medical/social services setting.
OR
- Associate degree and 2-4 years of experience working in a community based medical/social services setting.
OR
- Bachelor’s degree in social work or related field and 0-2 years of experience working in a community based medical/social services setting.
Case Manager II
- High school diploma/GED or equivalent is required and 6-8 years of experience working in a community based medical/social services setting.
OR
- Associate degree and 4-6 years of experience working in a community based medical/social services setting.
OR
- Bachelor’s degree in social work or related field and 2-4 years of experience working in a community based medical/social services setting.
OR
- Master’s degree in social work or related field and 0-2 years of experience working in a community based medical/social services setting.
Knowledge, Skills, and Abilities:
- Demonstrated cultural competency working with system-involved LGBTQ communities
- Knowledge of and familiarity with navigating social services and government agencies
