JOB SUMMARY:
The Home Care Supervisor is responsible for the overall compliance of a team of caregivers that service our individualized clients. The primary goals of the Home Care Supervisors are to maintain a case loads of client’s and ensure their care plans are being followed by the caregivers to provide the highest level of customer service and quality care. This position also assists with functions essential to the proper management of caregivers and client care. The Supervisor will follow the client through the intake process and continue to work diligently to maintain optimal, individualized care.
Responsibilities
· Provide regular contact with clients, including home visits, to ensure progress towards established goals.
· Ensures client’s schedules with caregivers are being maintained in accordance to their care plans.
· Client observation, intakes, and supervisory visits to the client homes.
· Assist with caregiver training and evaluations.
· Assist with client satisfaction surveys.
· Answer calls and respond to voicemails in a timely manner pertaining to each client/caregiver on caseload.
· Respond to emails and call centers in a timely manner.
· Maintain communication and a good rapport with the client’s case managers.
· Work collectively with the scheduling team to ensure optimal service to our client’s.
QUALIFICATIONS:
· High school graduate (or equivalent)
· Current Massachusetts HHA/Certified Nursing Assistant Certificate/License
· At least one-year experience in Home Health Supervisory/Leadership experience a plus
· Ability to clearly communicate with prospective and active caregivers via telephone, email, and in person
· Strong interpersonal relations and communication skills
· Able to work independently, demonstrating sound judgement, while following standards and guidelines
· Proficient with computer skills
· Ability to learn new management and scheduling software
· Good organizational skills, prioritization and multitasking
· Knowledge of common medical terminology is a plus
