As a Social Services Manager, you will serve as an advocate for residents and patients in a long-term care and short-term rehabilitation / sub-acute care setting, having administrative authority and accountability for the provision of their psychosocial needs. You will coordinate their plan of care, treatment goals, and discharge planning, as well as provide oversight of the center’s Social Services Coordinator(s). Qualifications:

• Bachelor’s degree or above in Social Work (BSW) or related field required, such as Human Services, Psychology, Sociology, Gerontology, Special Education, and/or Rehabilitation Counseling.

• Social Worker license in the state of employment a plus (i.e., LBSW, LSW, LCSW, LMSW, etc.).

• One (1 ) years’ supervised social worker experience working directly with individuals in a medical / health care setting required.

• Prior Long-Term Care, Skilled Nursing, or related healthcare social work experience strongly preferred
