Some duties that are required for this position need to be preformed by a certified/registered social service director in the state of LA. These job duties include but are not limited to:
-Case management/Discharge planning
-Conduct assessments according to CMS requirements
-Care plan implementation and evaluation
-End of life and Advanced care planning
-Assist residents with completion of benefit enrollments
-Assist residents and families with general services and admission into the facility
-Complete PASSR and 142 continued stay requests
Must be computer proficient, team oriented, caring, punctual, and innovative. Skills involving time management and organization will assist in completing all tasks this position entails.
