Social Services Director - Nursing Home in a nursing home environment, directs, establishes, and plans the overall policies and goals for the social services department. Responsible for patient assessments, care planning and helping the patients and families adjust to their new surroundings or occupations. Being a Social Services Director - Nursing Home plans and administers social service programs. Supervises the facility's social workers. Additionally, Social Services Director - Nursing Home assists in development of policies regarding participation in facility planning for health and welfare services. Requires a master's degree. Typically reports to top management. The Social Services Director - Nursing Home typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. Capable of resolving escalated issues arising from operations and requiring coordination with other departments.     Essential Duties and Responsibilities:
    • Functions as a Clinical partner with respective ComForCare business owner
    • Provides clinical oversight and case management as required by regulations or business line
    • Facilitates the orientation process, competency and skills validation process for caregivers
    • Provides supervisory oversight for a specific client, caregiver or field staff population as directed by regulations 
    • Provides education to team members, patients, family members of patients, caregivers, field staff, and referral sources
    • Manages caregivers and field staff while on assignment (clinical functions); assesses and investigates caregiver and field staff concerns and issues, and provides performance coaching, counseling and disciplinary action when appropriate.
    • Performing comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
    • Developing an individualized plan of care for each client that incorporates analysis of data and current scientific findings.
    • Maintaining client records showing systematic assessments, planning interventions, and evaluations.
    Minimum Requirements:
    • Active Registered Nurse (RN) license required 
    • Minimum one year experience as a Registered Nurse in NC/SC
    • Able to meet all federal, state, local contract and program requirements, in addition to internal certifications and training, as required
    • Excellent written and verbal communication skills
    • 2 years Supervisory or Management experience 
    • Proficiency with computers
