Summary
The Licensed Social Worker restores or enhances the social or psychosocial functioning of our patients by consulting with clients and their caregivers then assessing and planning referrals and interventions. LSW will act as a liaison when collaborating with other disciplines including caregivers, physicians, nursing, and other community resources to ensure complete quality care of the patient to maintain the health and overall quality of life through individualized advocacy and case management. The LSW will utilize knowledge of the patient’s age, medical history, background, and cultural diversity in the implementation of the patient plan of care.
