The schedule available is Wednesday to Saturday 0830-1900.

Responsibilities:

• Provide comprehensive nursing care to patients and families in a hospice or palliative care setting.
• Assess patient needs and develop individualized plans of care.
• Develop and implement strategies for symptom management and end-of-life care.
• Coordinate with other healthcare professionals to ensure continuity of care.
• Educate patients and families about the disease process, treatment options, and available resources.
• Provide emotional support to patients and families throughout the course of their illness.
• Document patient care activities in accordance with established standards.
• This position is a hands-on care, case manager who will be responsible for providing care to patients and families in a home setting.

Job Type: Full-time

Pay: $76,000.00 - $110,000.00 per year
