Staff Nurse - RN - Hospice jobs in Vermont
Staff Nurse - RN - Hospice provides professional nursing care to assigned terminally ill patients in a hospice environment. Records patient histories, performs various diagnostic tests, administers medications, and develops patient care plans in conjunction with other medical professionals. Being a Staff Nurse - RN - Hospice performs examinations and prescribed procedures, operating and monitoring medical equipment as necessary. Observes patients' condition and records observations in patient medical records. Additionally, Staff Nurse - RN - Hospice typically requires a bachelor's degree in nursing. Typically reports to a manager or head of a unit/department. Requires a valid state RN license. Staff Nurse - RN - Hospice's years of experience requirement may be unspecified. Responsibilities of Hospice Nurse Case Manager include:
- Assess, plan, implement, coordinate, and evaluate the care for each patient and develop appropriate treatment interventions, evaluation of outcomes, and utilization of services to ensure that patient care needs are met.
- Supervise other home care staff as necessary.
Qualifications of a Hospice Nurse Case Manager include:
- Must possess an active RN license in both VT and NH.
- ASN required, BSN or MSN preferred.
- Weekend and on-call rotation required.
