The Day Program Plan Coordinator (DPC) is responsible in assisting program management to ensure ISP Plans, Staff Action Plans and all corresponding documentation and timelines are met
A Day Program Plan Coordinator is responsible for the development of the individualized and person-centered program plans with the purpose of developing opportunities for self-determination, independence, increased productivity and greater community inclusion.
The DPC will develop plans to promote independence, community inclusion, individualization, and productivity while respecting the individual’s personal choice regarding lifestyle decisions and their ability to take informed risks.
The pay range for this position is $18.92-$19.97 and is commensurate on experience.

- Collaborate with your team to plan, create and assist with activities and opportunities to meet the needs and interests of the people you support.
Requirements:
- High School diploma or equivalent
- NYS Driver's License in good standing (some sites require this)
- Must be at least 18 years old and be able to lift a minimum of 50 pounds
