Primary Responsibilities:
The Clinical Leader/Health Services Director directly oversees the delivery of care to Assisted Living residents to ensure that the emotional, physical, psychological and safety needs are met through all program services. This includes medication management, staff coordination & training, infection control, and applying the nursing process to the care of seniors.
The successful candidate for this position will have:
Essential Functions:
Be able to lead the clinical team with a sense of purpose and accountability; utilizes metrics and established goals.
Oversees the medication management services, including training and audit of medication administration practices, collaboration with prescribing healthcare providers and pharmacy staff
Identifies ongoing needs and services of residents through assessment and service plans utilizing the nursing process
Visit prospective/current residents in their homes or healthcare setting as needed to assess for admission/readmission to the community.
Clinical liaison with families, residents, healthcare professionals and team members
Direct the infection control measures for the community
Utilize an EHR system for documentation, medication management and quality initiatives
Manage labor costs, supplies, and other line items in clinical budget.
Ensures that community follows all federal, state, regulations as it pertains to clinical care; including mandatory incident reporting to the State agency.
Responsible for collecting, analyzing, and reporting occurrence trends, and if necessary, develop/implement improvement plans.
Work collaboratively with others on the community leadership team
Provide, plan, schedule, and monitor staff training programs and in-services for the Assisted Living staff including statutory or regulatory required training. Attend all required training, in-service, and staff meetings.
Manage and coordinate the work of the clinical staff. This includes overseeing and/or completing staff schedules monthly and daily assignments.
Provide 24/7 on-call clinical support as needed.
